Brief clinical summary: The diagnosis of renal agenesis was made on routine ultrasound and confirmed by frusamide and repeat ultrasound. Labour was induced at 36 weeks gestation; it was breech presentation and delivered with forceps. The baby was born at 05.40 hours. It showed external features of potter’s syndrome, and the kidneys were impalpable. The baby was pronounced dead at 06.15 hours.
External appearances: The body is that of a female baby (weight 1,870 gms crown –heal length 43.5 cms, crown – rump 30.5cms, head circumference 32cm. and foot length 6.9cm. It shows some of the external features of potter’s syndrome; a large malformed, low set right ear, a beak like nose, redundant skin folds, hypoplasia of the finger nails and toenails.
Comment: The birth weight of 1,870 Gms is below the 10th centile for a gestation of 36 weeks.
Internal organs:
Respiratory system: The pleura, laryx, trachea and major bronchi appear normal; both lungs appear significantly hypo plastic and airless. (The total weight of both lungs is 14.59gms;the expected normal total weight of both lungs is approximately 38gms)The diaphragm is normally formed.
Cardiovascular system: The pericardium appears unremarkable. The heart is normal size, the heart chambers appear unremarkable. The foramen ovale is patent. As is the ductus arteries. The major blood vessels appear normally formed.
Gastro-intestinal system: The oesophagus, stomach, intestines, liver and pancreas appear unremarkable as do the thymus and spleen.
Endocrine system: The thyroid, adrenals and pituitary appear normal.
Genito-urinary system: Both kidneys are very small; the right kidney weighs 2.72gms and the left 2.73gms. (The expected total weight of both kidneys is approximately 16gms) Both kidneys contain small cysts. On sectioning, there is absence of normal cortical medullary demarcation. The appearance is those of bilateral severe renal dysplasia. Both ureters are present, and they drain normally into the urinary bladder. The uterus, both fallopian tubes and ovaries are present and appear normal.
Skeletal systems: Grossly normal.
Central nervous system: The skull is intact. The meninges appear unremarkable. The brain weighs 290gms.And appears unremarkable on multiple sectioning.
Placenta: This measures 16 X 12 cms. And weighs 270gms. The placental parenchyma appears unremarkable on multiple sectioning. The umbilicus is 22cms long and contains three blood vessels. The placental membranes appear unremarkable.
Histology:
Kidneys: Both kidneys show complete disorganization of the renal parenchyma. There are cysts, Bars of metaplastic cartilage, and primitive ducts. The appearances are those of renal dysplasia
Lungs: These show features of pulmonary hyperplasia. In addition there are extensive areas of diffuse intr- alveolar haemorrhage.
Umbilical cord: The presence of three blood vessels is confirmed microscopically.
Placental membranes: These show features of amnion nodosum.
Sections of the other foetal organs and of the placenta show no significant histological abnormality.
Final diagnosis:
Bilateral pulmonary hypoplasia,
Bilateral renal dysplasia.
